This intake asks for the information the Secretariat needs to understand the matter before deciding whether mediation, facilitation, referral or another route is appropriate. You can move back and review answers before signing and submitting.

Identity and verified contact

We verify the applicant email before a case can move forward. This reduces false submissions and gives the Secretariat a reliable contact channel.

MDM cardholder priority

Have an active MDM card?

An active MDM card gives the submission immediate MDM priority once the card is verified. Registered cardholder details can be prefilled securely, while eligibility, confidentiality and neutrality safeguards remain fully applicable.

Youth Mediator may request evidence of authority or guardian consent before the matter proceeds.

Route, parties and relationship

Tell us who is involved and how the matter sits within a personal, organizational, institutional or service relationship.

Facts, chronology and prior attempts

Give enough detail to understand what happened, what is disputed, and what has already been tried. Separate facts you observed from assumptions or second-hand information where possible.

Minimum 120 characters. Clear chronology helps the Secretariat assess the matter faster.

Safety, urgency and parallel proceedings

This stage helps prevent mediation from interfering with emergency protection, safeguarding duties, court orders, disciplinary procedures or other competent processes.

Emergency warning. Youth Mediator is not an emergency, police, medical or legal service. If there is immediate danger or an urgent legal deadline, contact the appropriate authority or professional first.

Outcome, boundaries and practical expectations

Mediation works best when the requested outcome is specific enough to discuss but flexible enough to explore alternatives.

Evidence and confidentiality

Upload only material that is relevant to understanding the matter. The Secretariat may request more information later if the case proceeds.

By signing below, I authorize the Youth Mediator Secretariat, within its programme mandate and safeguards, to receive, store, review, classify and administer this submission; verify information and documents where reasonably necessary; conduct eligibility, safeguarding and conflict-of-interest checks; communicate with me about the matter; assign authorized personnel if the case proceeds; and, where appropriate after internal review and applicable safeguards, contact or coordinate with identified parties or institutions for the purpose of mediation, facilitation, clarification, referral or case administration.

This authorization does not appoint Youth Mediator as my lawyer, legal representative or attorney, does not create a power of attorney, does not guarantee that another party will participate, and does not suspend any legal, contractual or administrative deadline.
